The Moise Safra Center Hebrew School is participating this year in a new competition called Jew Q. We will be joining two different schools on March 25th at New York Hebrew School at 121 W 19th Street New York, NY 10011. Departure from the Center at 3:50 pm. Arrival back to the Center 6:45 pm Person(s) in Charge: Frumie Weitman and Chana Matussof

1. I have been informed of the details of this educational field experience.
2. My child has my permission to participate in this supervised field experience.
3. I agree to instruct my child to obey all rules, regulations and instructions given by teachers and/or authorized school personnel. I further agree that the Center or no teacher or authorized personnel shall be held responsible or liable for injuries or other mishaps caused by my child’s deliberate disobedience of rules, regulations or instructions.
